  1. Completed Application, via SurveyMonkey by Feb. 15, 2024
  2. $25 Application Fee
  3. One letter of recommendation sent to PO Box 11757,                    Charlotte, NC 28220 or emailed by your recommender to           info@leadershipcharlotte.org
  4. Completed the Applicant Commitment Form
  5. Completed Employer Commitment Form by direct supervisor
